Once on the Next page, you will be asked to input your website, your email, and that you should click some tiny box if you will like them to keep sending you promotions emails straight into your inbox. So go ahead and provide your details and click on the SAVE AND CONTINUE button below. If you click on that button, you will be taken to the next page.
On the Next page, just choose your country, read and accept their policy or privacy or user agreement. and click on accept to move to the next page.
On this page, you have to be very careful, because you will need to supply them your real name and a valid address. Why is this important? its important because before you get paid by Google, you will need to prove your the address you provided is valid. Google will send a physical mail containing a pin to that address, you get the mail and use the pin to verified your Adsense Account.
